Smart119 Inc. Smart119 Inc. (Smart 119) won the "Next Emergency Information Sharing System Lease" tender conducted by Chiba City, Chiba Prefecture, for the purpose of "changing today's "119"(119 is the number for emergency calls in Japan) and concluded a contract on April 24 this year. The emergency information sharing system "Smart119" will be put into operation at the actual emergency medical treatment site from July 20, following the test operation.
Smart119 was developed through a research and development project (project cost: 275 million yen, 2016 - March 31, 2019) funded by the Japan Agency for Medical Research and Development (AMED), in collaboration with the Graduate School of Medicine, Chiba University, in response to the needs of emergency and fire departments.
The field of emergency medicine faces many challenges, and we aim to solve them with advanced technology and creative ideas to provide reliable emergency care. The number of emergency calls to 119 is on the rise, and the need for efficiency and optimization is becoming a common social issue throughout Japan.

One of the major problems is that the flow from the 119 call to the emergency team rushing to the hospital is an analog relay, which results in information restriction and confusion. This also leads to delays of transferring patients efficiently.
Despite the advancements in information technology, this problem is still unsolved in the emergency department.

"Smart119" enabled information on 119 calls to the Chiba Fire Department Joint Command Center to be entered automatically into a tablet device, at the same time immediately checking the hospital availability and to be shared with emergency crews. This system helps to reduce the inefficient transportation of patients, and ensure they get to the right medical institutions quickly.

Smart119 differs from existing systems in that patient information can be entered automatically via voice recognition as well as via a touch screen or keyboard on a dedicated Android tablet device. By picking up the voice from the operator at the firefighting command center, we have succeeded in reducing the time required to enter information by about 80%.

◆AI-based "predictive diagnosis" algorithms are nearing completion

Another feature of "Smart119" is the development of a "predictive diagnosis" algorithm that utilizes artificial intelligence (AI), which is not available in other systems. In the case of serious diseases such as stroke or acute myocardial infarction, it is important to select a specialist hospital as the destination. The algorithm based on data collected so far with the cooperation of the Chiba City Fire Department has made it possible to predict ROC curves (Receiver Operating Characteristic Curve) with a high accuracy of AUC (Area Under Curve) = 0.881. Development is underway with the goal of implementing the algorithm in Smart119 by fall of this year.
Dr. Taka-aki Nakada, CEO of Smart119 Inc. and Professor of Emergency Intensive Care Medicine at Chiba University Graduate School of Medicine, said: "After four years of development, Smart119 has finally been installed in ambulances and is now playing an active role in the field of medicine, which gives me a great sense of accomplishment."
In the future, we will not only spread the use of Smart119 across the nation's emergency medical services, but also promote the practical application of AI prediction and the development of a 'personal application' that stores information on regular medication and pre-existing conditions with the patient's permission and communicates this information to the emergency services and hospitals in case of emergency.
We aim to make a contribution for better acute care.
